Thanks for sharing your story. Please read this before your interview, then sign the release form we send you.

I agree to be interviewed and recorded by Mistor Testimonials (“Mistor”), on behalf of the coach or business who introduced me (“the Business”).

1. Permission. I give the Business and Mistor permission to record, edit and use my name, image, likeness, voice and statements from this interview (“the Recording”), in whole or in part, in any media, worldwide, with no time limit. This includes websites, social media, email, sales pages and paid advertising.

2. Editing. I understand the Recording may be shortened, captioned and combined with graphics, music and branding. Edits will not change the meaning of what I said.

3. Portfolio. Mistor may show the edited clips in its own portfolio only if the Business allows it.

4. Honesty. My statements are true and reflect my own experience. I understand my results are my own and may not be typical.

5. No payment. I am not being paid for this, apart from any thank-you gift offered.

6. Age. I am 18 or older.

The form asks for your full name, email, the coach or program that introduced you, and a checkbox to confirm you agree.
